We will collect the following types of information from you or about you from a third party (provider organisation) engaged in the delivery of your care:

‘Personal data’ meaning any information relating to an individual that can be directly or indirectly identified from the data. This includes, but is not limited to, name, date of birth, address with postcode, telephone numbers and next of kin.

‘Sensitive data’ such as medical history including details of appointments and contact with you, medication, emergency appointments and admissions, clinical notes, treatments, results of investigations, supportive care arrangements, social care status, race, ethnic origin.

Your healthcare records contain information about your health and any treatment or care you have received previously from us at Novaselves Ltd Services, but may also contain records from any health care provider that we have referred you to or that you have volunteered to give us to keep on file (e.g. from an acute hospital, NHS GP surgery, community care provider, mental health care provider, walk-in centre, social services). These records will be in electronic format – we do not keep paper copies of medical data at this clinic.

We use a combination of technologies and working practices to ensure that we keep your information secure and confidential.

3.1 Personal Data is processed strictly in accordance with GDPR and Schedule 2 of Data Protection Act. In most circumstances Personal Data is processed based on consent by the data subject to do so, however we may also process Personal Data:

d) New business owners: Should Novaselves Ltd Services be acquired by a third party; we will share your Personal Data with the new owners of the company and their professional advisors.

6. How long do we hold your data?

We will only retain your Personal Data for as long as is necessary for the purpose(s) for which the data was collected. We will review the purpose for which the Personal Data is collected for to determine how long we will need to keep it, and securely delete your information when it is no longer needed for this purpose. Any information that goes out of date will be updated, achieved or securely deleted.

Furthermore, where the processing of Personal Data is subcontracted, we make efforts to protect such Personal Data by selecting subcontractors who handle Personal Data

appropriately and by prescribing the necessary measures to the subcontractor for the appropriate data management.
